Abstract

Glycogen storage disease type III results from a deficiency of the enzyme, amylo-1,6- glucosidase, which is responsible for the breakdown of debranching of the glycogen molecule during catabolism and goes with hypoglycaemia, muscle weakness and endorgan dysfunction due to glycogen accumlation. Primary problems for anaesthesiologist are hypoglycaemia, liver dysfunction, delayed anaesthetic recovery, excessive surgical bleeding and cardiomyopathy. We present the anaesthetic management of a patient with glycogen storage disease type III scheduled for orthopedic surgery.

Glikojen depo hastalığı tip III, glikojen molekülünün yıkımı sırasında yan bağların kopmasından sorumlu amilo-1,6-glikozidaz enzim eksikliği sonucu gelişen ve hipoglisemi, kas güçsüzlüğü ve organlarda glikojen birikimi ile seyreden bir hastalıktır. Bu hastalarda anestezist için primer problemler hipoglisemi, karaciğer fonksiyon bozukluğu ve buna bağlı olarak anesteziden uyanmada gecikme, aşırı cerrahi kanama ve kardiyomiyopatidir. Bu yazıda glikojen depo hastalığı tip III ve buna bağlı karaciğer sirozlu bir olguya, ortopedik cerrahi işlem sırasında uygulanan anestezik yaklaşım sunulmuştur.
