Although pulmonary atelectasis is postoperatively common, severe life-threatening intraoperative atelectasis is very rare. We report on a case with acute intraoperative atelectasis and profound hypoxemia, and discuss the management of this condition. A patient undergoing general anesthesia in the flank position for renal surgery suddenly had decreased oxygen saturation approximately 1 h after the induction of anesthesia. Hypoxemia continued despite manual ventilation with 100% oxygen. Following surgery chest X-ray showed collapse of the left lung. Broncoscopy was performed with saline lavage. The patient was transferred to the intensive care unit, and extubated 12 h later. Although uncommon, massive atelectasis should be considered in the differential diagnosis of peroperative hypoxemia.

Postoperatif pulmoner atelektazi sık karşılaşılan bir olay olmasına rağmen, hayatı tehdit eden intraoperatif atelektazi oldukça nadirdir. İntraoperatif atelektazi nedeniyle hipoksemi gelişen bir olgumuzu bildirmeyi ve bu konuyu tartışmayı istedik. Genel anestezi altında Flank pozisyonunda sol renal tümör nedeniyle nefrektomi uygulanan hastada anestezi indüksiyonundan yaklaşık bir saat sonra ani olarak oksijen saturasyonu düştü, basınçlı ventilasyon ve %100 oksijene rağmen hipoksemi devam etti. Operasyon sonunda çekilen akciğer grafisinde sol akciğerin tamamen atelektazik olduğu görüldü. Bronkoskopi ve lavaj yapıldı, yoğun bakıma alınan hasta postoperatif ikinci gün atelektazinin düzeldiği görülerek ekstübe edildi. İntraoperatif hipokseminin ayırıcı tanısında çok sık görülmemesine rağmen massif atelektazi de düşünülmelidir.
