Herein, we report a 22 year-old female patient who had complaints of petechiae at the lower extremity, ecchymoses, hematuria and menorrhagia and history of premature birth in the 32nd week of her pregnancy. Initial tests revealed that Prothrombin Time, International Normalized Ratio and Activated Partial Thromboplastin Time were 19-85s (N: 12-16), 1.92 (N: 0.95-1.5) and 73.5s (N: 26-46), respectively. Factor II 35.4% (N: 70- 120), Factor V 67% (N: 70-140), Factor VII 61% (N: 170-140), Factor IX 33.51% (N: 70-120), and Factor X 62% (N: 70-120) were detected. Anticardiolipin antibody IgG was 60U/ml, Ig M was 40U/ml and positive lupus anticoagulant levels were detected. In order to confirm the presence of lupus anticoagulant antibodies, the coagulation tests were repeated after adding phospholipids into the test environment. It was observed that the prolonged tests were improved. Hemorrhagic diathesis rarely appeares in Antiphospholipid Syndrome, and was explained by the observed significant low levels of Factor II and Factor IX in the present case.

Yirmi iki yaşında alt ekstremitede petesi, ekimoz, hematüri ve menoraji yakınmaları ile başvuran kadın hastanın gebeliğinin 32. haftasında prematür doğum hikayesi vardı. Protrombin Zamanı: 19.8 sn (N: 12-16), International Normalized Ratio: 1.92 (N: 0,95-1,5) ve aktive Parsiyel Tromboplastin Zamanı: 73.5 sn (N: 26- 40) tespit edildi. Faktör II: % 35.4 (N: 70-120), Faktör V: % 67 (N: 70-140), Faktör VII: % 61 (N: 70-140), Faktör IX: % 33.51 (N: 70-120), Faktör X: % 62 (N: 70-120) olarak rapor edildi. Antikardiolipin antikor IgG: 60 Ü/ ml, IgM: 40 U/ml ve lupus antikoagülan pozitif bulundu. Lupus antikoagülan antikorlarının varlığını doğrulamak için koagülasyon testleri ortama fosfolipid eklenmesinden sonra tekrarlandı ve uzamış olan testlerin düzeldiği görüldü. AFS'nda çok nadir görülen kanama diyatezi, olgumuzda Faktör II ve IX aktivite düzeylerinde izlenen belirgin düşüklük ile açıklanmıştır.
